RuralCare Connect™ is a grant-funded community health program making primary, specialty, and ongoing care more accessible for most residents of Southern New Jersey.

We bring free health screenings to communities across Southern NJ. These events are a simple, low-barrier way to check in on your health, connect with a care team, and find out if there’s anything worth following up on. Screenings may include:

RuralCare Connect is open to most residents of Atlantic, Burlington, Cape May, Cumberland, Ocean, and Salem counties who face barriers to consistent medical care. You may be a good fit if:

A lot of New Jersey communities — even ones that don’t feel remote — sit far from the doctors and resources that keep people healthy. Those who need care aren’t always able to make it happen.
Distance, busy schedules, limited transportation, and a healthcare system that isn’t always easy to navigate can all get in the way.
RuralCare Connect was created to remove those barriers.
We connect residents across southern New Jersey with primary and specialty care services through community health screenings, telehealth visits, and personalized care coordination, so that where you live, or your ability to navigate technology or the medical system, are never the reason you go without care.
RuralCare Connect is offered by Deborah Heart and Lung Center, a specialty hospital in Browns Mills, NJ that has been serving patients across New Jersey and beyond for over 100 years. Since 1922, Deborah has cared for more than 2.3 million patients and that commitment to access and community drives everything RuralCare Connect does.
New Jersey may not look rural on a map, but many of its communities — spread across large counties, far from major medical centers — face the same access challenges as rural areas across the country. The government recognizes this and developed grants to fund programs that make healthcare more accessible. That’s how RuralCare Connect came to be.
If you live in one of the areas highlighted in blue on our coverage map, this program is for you, regardless of how you’d describe the area you call home.
Financial Responsibility
At no cost to you, as a benefit of state funds, the RuralCare Connect team at Deborah will help you with:
The assistance of our RuralCare Connect team with these services is free to you, as well as the remote monitoring tools and in-home visits provided by our DocGo® partner. However, payment for other testing, appointments and treatments that we help arrange for you with other doctors and facilities will be billed to you or your insurance company however they are currently handled.
The RuralCare Connect team will schedule your appointments — either in-office, via telemedicine, or in your home. If you have a telemedicine appointment, you can use our state-of-the-art equipment to participate in the call and our medical team will be available to measure your vitals and share them with your doctor in real time.
If you live in most areas of Atlantic, Burlington, Cape May, Cumberland, Ocean, and Salem counties, your area qualifies as rural based on census data, and therefore you are eligible. It doesn’t matter what type of place you call home (apartment, townhouse, farm, neighborhood, etc.), you qualify based on the township you live in.
The DocGo® mobile team can provide primary care services in your home for checkups, illnesses, wound care, prescription management, referral purposes and more.
You may also qualify for in-home remote monitoring services if you have chronic issues like diabetes, cardiovascular disease, hypertension, cancer, and chronic respiratory diseases.
We regularly host free health screenings and community wellness events in neighborhoods across New Jersey. Find an event near you, and come as you are — no referral, no paperwork, and no preparation needed.

Connect with local police, firefighters, first responders, and community organizations while enjoying family-friendly activities, interactive displays, a Touch-a-Truck experience, and health screenings in a celebration of community and public safety.
